Leitsymptom aller Le-Fort-Frakturen:

Zähne von Ober und Unterkiefer passen beim Beißen nicht ineinander

Reposition der Frakturfragmente unter Einstellung einer Okklusion

-> CAVE aufsteigende Entzündung Meningitis
Diagnostik:
Liquorrhinorrhoe
ß2-Transferrin testen im Ausfluss
ß-Trace-Protein

bei nicht o. nur minimal dislozierten Frakturen ohne neurologische Verletzungen

insb. bei positiven Liquorrhinorrhoe-Nachweis
Darstellung und Reposition der Fraktur

offene neurochirurgische Duraplastik
Laterale Mittelgesichtsfrakturen (Jochbein- und Jochbeinbogen-Fraktur)

Ätiologie:
Trauma bei Sport oder Autounfällen
Kieferklemme (beeinträchtigte Mundöffnung)
Kiefersperre (beein. Mundschluss)
Sensibilitätsausfall im Bereich N. infraorbitalis
Augenbeteiligung
Monokelhämatom
Diplopie
Konservativ bei keiner Dislokation oder Funktisonbeeinträchtigung in Umgebung
Operativ bei dislozierten oder nicht-reponierbaren Frakturen, motorische Funktionseinschränkungen der Augenmuskulatur/ Sensibilitätseinschränlkungen
Frakturreposition und osteosynthetische Fixation
Nasenfraktur
stumpfe Gewalteinwirkung von frontal oder lateral
Symptomatik:
häufig Nasenbluten (Epistaxis)
Behinderung der Nasenatmung
Schwellung und Schmerzen der Nase
sichtbare Deformation der Nase
Klinische Untersuchung:
Schiefstand der Nase
Ipsilaterale Impression, Stufenbildung der gegenüberliegenden Nasenwand
Krepitationen beweisend
Anteriore Rhinoskopie:
Inspektion Naseneingang
Ausschluss Septumhämatom
Apparativ:
CT
nicht-disloziert, geschlossene Fraktur
abschwellende Maßnahmen
offene Fraktur
dislozierte, geschlossene Fraktur
Reposition in zwei Ebenen
Anlage Nasengips
Endonasale Tamponade
Komplikationen:
Schwere Epistaxis
Septumhämatom
Felsenbeinfraktur
